it really doesn't matter if you have a inner hand grab it depend on the speed of which you hit the joint and the force applied. for example if you have a bent knee and you kick it, it doesn't always break it (the foot is planted on the ground, knee is bent ) because you either don't have enough speed to apply the required force or you hit it from the wrong angle
well if there is a lot of stress kn the wrist and you extend it it can sometimes come off. When you see the other wrist kinda come off the arm without DMing and you extend you wrist it can sometimes come off. Or it will lift them. it really depends on the situation. Also when your hands kinda merge together when they are both in grab then when someone lets go there hand can pop off
