This appeared after surgery on my right knee.
Occasionally it will ooz out anasetic and will puff up.

Probably something you should consult a doctor about, to be safe, even if you don't know.
From the picture it's pretty hard to tell, but the area seems a bit swollen, with a small bit of discolouration so I would guess it most likely is.

How long has it been there? To me it looks like a scab was forming, but the healing process has failed. Chances are it has been re-infected from poor care. Re-infections after surgery is not uncommon, but it doesn't look severe so nothing to stress over. Just as long as you go back to the doctor. It's always the small problems that turn big.
