This is a question I've always wanted to ask.

I see that some avatars are 150x200 pixels (e.g. BenDover, Dad, Tryton, etc.) and if I'm not mistaken (no, I'm not mistaken at all), the maximum size for avatars are 150x150 (for TP users) and 100x100 (normal users).
Are these bought or is it a prize or something?

Don't know about the others

They can be bought but don't be surprised by very high prices (eg 250k mentioned in above post). Not all admins like to be bothered by such requests. Tryton got it as a gift for his past contributions towards unibash. Dad and shade bought it. Not quite sure about BenDover.

People either blew a stupid amount of TC, or got them as a reward for doing stuff.
